ATC- Awarding Certification

Pet Emergency Education requires proof that a class attendee successfully completed the training and earned the certification we provide. The Attendance and Assessment Verification Form is a document that lists all of the class participants names which have attended the class and been tested by the instructor.  It is a very important way we can prove a person showed up for the class and accomplished all of the requirements to become certified in pet CPR and first aid. Without this form, we can not issue certificates to your participants.
